REGISTRATION IS A 3-STEP PROCESS

Because camp reservations are first come first serve, all 3 steps of the registration process must be complete for your student to "reserve a spot." The price paid for every individual student is dependent on the completion of all 3 of these steps. Incomplete forms and a delay in submitting the required documents will result in a resubmission fee of $10.

Step 1: Complete Online Registration

Click the icon and follow the steps to register. Enter YOUR EMAIL at the end and complete Step 2.
Information Needed:
  • Camp Attending: Camp THREE
  • Group Name: Dominion Church - Dickinson
  • Passcode (if needed): dickinson26
  • A deposit of $129 is due at registration. This reserves your student's spot and is non-refundable. 
    • Please note: the $129 deposit will come off of your total balance due.
  • Pre-order merch is available until April 23. This is an additional cost and must be paid digitally. 

Step 2: Complete Online Consent Forms

In addition to registation forms, every student is also required to complete a digital Camper Liability/Participation Form through Carolina Creek/Camp Cho-Yeh 
  • All minors must have a legal guardian complete and sign this form
  • Please Note: The Youth Pastor is not permitted to sign these forms on behalf of a parent or guardian
Please make sure this form is completed before check-in on June 29th.

Step 3: Pay remaining balance

Please be sure to select or note “Youth Camp” when submitting your payment so it’s applied correctly.

Print and fill out this form to turn in the morning we leave for camp. THIS FORM WILL NOT  BE ACCEPTED UNTIL THE MORNING WE LEAVE FOR CAMP.

D.C. LIABILITY RELEASE FORM

This only has to be filled out once a year. You do not have to fill this form out if you have already done so this year.
